OBJECTIVE: The purpose of the study was analise the results of mitral valve repair with the double teflon technique: quadrangular resection of the posterior leaflet, annulus plicature with "pledgeted" suture over teflon patch and suture of the leaflet.

METHODS: Seventy- two patients with mitral insufficiency due to myxomatous degeneration with ruptured or elongated chordae submitted to mitral valve repair with this technique were evaluated in the immediate and late postoperative period. The mean age was 60,5 +/- 11,9 years and 56,9% were male. Fifteen patients (20,8%) were submitted to associated procedures .

RESULTS: There was one immediate mortality (1,4%). In the late postoperative period, 66 (91,7%) patients were in functional class I (NYHA). The linearized rates for death, reoperation and thromboembolism were 2,6%; 1,3 and 2,6% per patients-year, respectively. There were no episodes of hemolysis or endocarditis. The actuarial survival was 94,5% +/- 3,2% at six years. In the echo-Doppler evaluation, we found significant decrease in the left atrium diameter (p=0,0001) and in the left ventricular diastolic diameter (p=0,0003).

CONCLUSION: Mitral valve repair with double teflon technique presented low operative mortality, satisfactory survival rates and excellent clinical evolution.
